Puis, la courbe finit par s\u2019inverser et le syst\u00e8me de soins de sant\u00e9 parvient \u00e0 d\u00e9passer le nombre de patients. Tout cela se fait normalement sur des dur\u00e9es tr\u00e8s courtes. Ici, avec le Covid-19, nous sommes sur des dur\u00e9es beaucoup plus longues. Cela demande beaucoup de r\u00e9silience de la part des \u00e9quipes.<\/span><\/p>\n<h3><b>En voyant les images de la situation en Italie, avez-vous craint de vivre pareille situation ?<\/b><\/h3>\n<p style=\"text-align: justify;\"><b>Yves Maule :<\/b><span style=\"font-weight: 400;\"> Oui et non, notamment parce que le fantasme est souvent bien plus grave que la r\u00e9alit\u00e9. Les images que nous avons vues sont des images extr\u00eames. Tous les h\u00f4pitaux italiens n\u2019\u00e9taient pas dans cette situation-l\u00e0. Nous avions \u00e9videmment les images italiennes quelque part dans un coin de la t\u00eate. Nous avons sans doute mis la pression sur nos \u00e9quipes au cas o\u00f9, tout en priant pour que cela n\u2019arrive pas. Dans nos papiers, nous avons des sc\u00e9narios pr\u00e9vus, des protocoles qui ont \u00e9t\u00e9 discut\u00e9s en amont pour ne pas devoir le faire au chevet du patient. Si elles doivent arriver, les choses devront \u00eatres faites. Personne ne viendra les faire \u00e0 notre place.<\/span><\/p>\n<p style=\"text-align: justify;\"><b>David De Bels :<\/b><span style=\"font-weight: 400;\"> Nous avons d\u00fb anticiper les d\u00e9cisions pour \u00e9viter la pr\u00e9cipitation. Nous avons \u00e9galement demand\u00e9 que notre comit\u00e9 d\u2019\u00e9thique ait une cellule d\u2019urgence joignable 24h\/24.<\/span><\/p>\n<h3><b>En cas d\u2019augmentation du nombre de patients en soins intensifs, on \u00e9voque d\u00e9j\u00e0 beaucoup la question des choix \u00e0 faire, comment y r\u00e9pondre\u2009?<\/b><\/h3>\n<p style=\"text-align: justify;\"><b>David De Bels :<\/b><span style=\"font-weight: 400;\"> Pour l\u2019instant, le choix n\u2019est plus vraiment celui des admissions dans notre service. La discussion actuelle porte surtout sur la question de savoir jusqu\u2019o\u00f9 aller dans nos traitements lourds et invasifs aux soins intensifs. Qui sont les patients qui vont en b\u00e9n\u00e9ficier ? Certaines techniques extracorporelles sont limit\u00e9es par le mat\u00e9riel disponible et donc nos indications sont parfois plus restrictives car nous savons que nous pourrions \u00eatre en difficult\u00e9 si un afflux trop important de patients devait arriver.<\/span><\/p>\n<p style=\"text-align: justify;\"><b>Yves Maule <\/b><span style=\"font-weight: 400;\">: Mais la situation actuelle est diff\u00e9rente d\u2019une catastrophe. Si on reprend les attentats du 22 mars 2016, durant lesquels les \u00e9quipes de Brugmann \u00e9taient dans postes m\u00e9dicaux avanc\u00e9s, l\u00e0, des choix ont d\u00fb \u00eatre faits sans avoir le temps d\u2019y penser. Nous \u00e9tions dans un contexte de prise de d\u00e9cision dans un laps de temps tr\u00e8s court. Ici, nous avons du temps pour prendre les d\u00e9cisions \u00e0 plusieurs, cela permet notamment de pr\u00e9parer l\u2019environnement. Pour les \u00e9quipes, il y a forc\u00e9ment des liens avec les patients. M\u00eame s\u2019ils sont endormis, il y a des contacts. Celle-ci discute avec les familles et nous donne un feedback. Quand on les appelle, nous avons d\u00e9j\u00e0 une id\u00e9e globale de la situation, ce qui nous permet d\u2019adapter le message. Si la situation n\u2019est pas bonne, il faut y aller progressivement. Il faut pouvoir anticiper, tout en restant honn\u00eate. Communiquer avec les familles uniquement par t\u00e9l\u00e9phone complique par contre encore plus la situation.\u00a0<\/span><\/p>\n<p style=\"text-align: justify;\"><b>Yves Maule : <\/b><span style=\"font-weight: 400;\">L\u2019accompagnement du patient qui s\u2019en va, c\u2019est aussi une forme de soin. Ces familles qui ne peuvent pas voir leurs proches en fin de vie, cela repr\u00e9sente \u00e9galement une forme de souffrance pour nos \u00e9quipes.<\/span><\/p>\n<h3><b>Apr\u00e8s trois semaines, comment est le moral de vos unit\u00e9s\u2009?<\/b><\/h3>\n<p style=\"text-align: justify;\"><b>Yves Maule : <\/b><span style=\"font-weight: 400;\">La premi\u00e8re semaine a \u00e9t\u00e9 tr\u00e8s compliqu\u00e9e. Il a fallu s\u2019adapter au chaos. D\u00e9sormais, vivre au coeur de celui-ci est devenu le mode de fonctionnement commun. Les gens s\u2019adaptent et le moral reste bon. Nous avons eu la chance de ne pas avoir de coll\u00e8gues gravement malade. Heureusement. Les gens restent assez positifs pour le moment, mais si la situation devait durer deux ou trois mois, je ne sais pas si tout le monde sera capable de tenir. Un suivi psychologique est d\u00e9j\u00e0 possible pour les \u00e9quipes. Certains ne dorment sans doute pas beaucoup la nuit. Heureusement, nous avons pris la bonne d\u00e9cision de permettre \u00e0 tout le monde de pouvoir souffler pour tenir le coup et se projeter un peu plus loin. Si tu es tout le temps dedans, ta t\u00eate reste ici et tu ne prends jamais de recul. Nous aurons aussi un gros travail d\u2019accompagnement \u00e0 effectuer quand tout \u00e7a sera termin\u00e9. Apr\u00e8s quinze jours de r\u00e9animation lourde, ces gens sont compl\u00e8tement ext\u00e9nu\u00e9s et vont avoir une p\u00e9riode post-soins intensifs tr\u00e8s importante. Il faut g\u00e9rer le stress post traumatique chez les patients et g\u00e9rer leur revalidation. Chaque jour, ces gens perdent environ 1% de masse musculaire en \u00e9tant alit\u00e9 aux soins intensifs. C\u2019est long et compliqu\u00e9 \u00e0 reconstruire. Sortir les patients des soins intensifs, c\u2019est notre premi\u00e8re mission, mais cela ne suffit pas pour leur permettre de retrouver une vie normale.<\/span><\/p>\n<p style=\"text-align: justify;\"><b>Yves Maule : <\/b><span style=\"font-weight: 400;\">Et puis il y a tous ces patients \u00ab\u2009non Covid\u2009\u00bb qu\u2019il faut continuer de soigner avec le m\u00eame degr\u00e9 d\u2019exigence. Pour le moment, ils ont peur de venir \u00e0 l\u2019h\u00f4pital. Dans les mois qui viennent, il faudra tirer les le\u00e7ons de ce qui a fonctionn\u00e9 ou non et r\u00e9ajuster le tir. D\u2019accord, il faut faire des \u00e9conomies, mais faut-il encore les faire sur le secteur de la sant\u00e9 ? Nous savons qu\u2019une \u00e9pid\u00e9mie risque de revenir. Quand et sous quelle forme? Impossible de le pr\u00e9dire. Ce serait dommage de ne pas tirer les bonnes conclusions,\u00a0 de ne pas mettre en place les structures qui permettent de monter en puissance au niveau des soins de sant\u00e9.
